Individualized Transition Skills Program (ITSP) aims to improve the quality of life for Transition Age Youth by providing responsive, efficient, and high-quality public services that promote self-sufficiency, well-being, and financial security of individuals within their communities.
Our Services
Through our ITSP program, we provide personalized, in-home assistance to foster youth, helping them achieve stability and independence. By offering consistent support and resources, we are improving educational success and reducing the likelihood of homelessness among foster youth.​

Education
We assist clients in checking their credits to ensure they are on track to graduate, help with credit recovery, and provide resources for tutoring, if needed. We also guide them in setting career goals, applying for FAFSA, and completing applications for technical programs, community colleges, or universities.

Employment
We help clients obtain vital documents (CA ID and SSC), complete job applications, create resumes, and prepare for interviews by providing clothes and practicing communication skills. We also offer tips on time management, completing hiring paperwork, and job retention strategies.

Housing Stability
We assist clients with LA County transitional housing applications, provide thorough guidance on apartment hunting, offer insightful advice on selecting the right roommate, and connect them with essential resources to ensure a successful and stable transition out
of care.

Health and Well-Being
We assist clients in keeping their Medi-Cal up to date, accessing vital mental health resources, and building supportive daily practices that promote overall health and well-being, ensuring they have the tools needed for a healthier and more stable future.

Self-Advocacy
We help clients fully understand their rights, build a strong network of supportive adults, and expand their knowledge of valuable community resources, empowering them with the essential tools and connections they need to thrive as they transition into adulthood.
